Information about your first visit to Welcome Pediatrics

1. Please call Welcome Pediatrics at (718) 258-9888 to schedule your appointment.
   
   
2. In order to minimize paperwork at the office, you can download and fill out the necessary documentation right at home. The forms that need to be filled out are available in our Patient Forms Section. They include: Patient Registration, Family Health History, and an Authorization for Information Release. Please fill out the Authorization for Information Release only if you are coming to us from another doctor. This form allows us to obtain medical records from the patient's previous medical provider.
   
   For your convenience the forms are available in PDF format and are able to be filled out electronically. You can also print them and fill them out by hand. Please complete the forms to the best of your ability. If you are unsure of something, we will be more than glad to help you in the office during your first visit.
   
   
3. Review the HIPAA privacy policy and make sure you understand it. At Welcome Pediatrics, patient confidentiality is very important and is taken extremely seriously. If you have any questions about your privacy rights, we will be happy to answer them.
   
   
4. Bring your insurance card and identification card. We will need to confirm your insurance and your eligibility.
